Remote Patient Monitoring is a CMS created program using connected devices to manage patients between visits to their provider.

RPM uses diagnostic devices and communication technology to transmit patient data and provide actionable information for providers.
Patients with one or more of the over 1100 CMS designated diagnosis codes are eligible to have this program added to their care plan by their provider.
These patients are provided with a device that measures such data as blood pressure, weight, pulse oximetry, blood glucose and many more. This data is transmitted into our Carelinc software and monitored by our Care Coordinators. Providers determine the acceptable thresholds by data category and by patients. When readings fall outside these thresholds, our care team relay this information to providers. We monitor, providers manage.
This data can be collected as often as deemed appropriate by providers.
